    摘要:目的:评价医务人员高尿酸血症(hyperuricemia,HUA)与代谢综合征(metabolic syndrome,MS)的相关性,研究HUA发生的危险因素,为通过改善生活方式降低HUA发生风险提供依据。 方法:以北京某医院参加职工体检的2 763名医务人员为研究对象,建立健康人群血尿酸(UA)的参考区间;用logistic回归分析HUA发生的风险因素,评价MUA与MS的相关性。 结果:男性血UA的参考区间为244~496 μmol/L;女性为175~398 μmol/L(≤50岁)、183~415 μmol/L(>50岁)。超重(OR 2.312,95%CI 1.840~2.905)、血压升高(OR 1.902,95%CI 1.470~2.463)、三酰甘油(TG)升高(OR 1.765,95%CI 1.372~2.272) 及MS(OR 3.906,95%CI 3.052~5.001)是HUA发生的独立危险因素。UA水平随MS诊断指标异常个数的增多而升高(P<0.05),每增加一个异常指标,HUA的患病风险升高1.723倍(95%CI 1.567~1.896)。 结论:本研究建立的医务人员血UA参考值较先前报告的有所升高,MS是HUA的独立危险因素。

    Abstract:

    Abstract: Objective:To evaluate the correlation between hyperuricemia and metabolic syndrome in medical staff and the risk factors resulted in hyperuricemia, and provide the evidences for reducing the risk suffering from hyperuricemia by improving the lifestyle. Methods:A total of 2 763 medical staff who received the physical examination in the same hospital in Beijing were enrolled in this study. Based on the levels of serum uric acid of participants, the reference interval of serum uric acid in healthy subjects was set up. The risk factors resulted in hyperuricemia and the relationship between hyperuricemia and metabolic syndrome were analyzed by the univariate and multivariate logistic regression analysis. Results:The reference intervals of serum uric acid for males and females younger than 50 years of age (including 50 years old) were 244 to 496 μmol/L and 175 to 398 μmol/L respectively, and the females older than 50 years were 183 to 415 μmol/L. The multivariate logistic regression analysis indicated that overweight (OR=2.312, 95% CI 1.840 to 2.905), high blood pressure (OR=1.902, 95% CI 1.470 to 2.463), elevated triglycedies (OR=1.765, 95% CI 1.372 to 2.272) and metabolic syndrome (OR=3.906, 95% CI 3.052 to 5.001) were all the independent risk factors resulted in hyperuricemia. The serum uric acid levels rose with increased abnormal criteria for diagnosis of metabolic syndrome (P<0.05), and the risk suffering from hyperuricemia increased by 1.723 times (95% CI 1.567 to 1.896) for each additive abnormal criteria. Conclusion:The reference interval of serum uric acid established in this investigation was higher than that in the previous reports. MS may be the independent risk factors resulted in hyperuricemia.
