    摘要:目的:研究特发性血小板减少性紫癜(ITP)患者外周血中调节性B细胞(regulatory B cells, Breg)亚群和相关细胞因子的变化及其相关性,探讨Breg在ITP发病机制中的作用。 方法:选择30例ITP患者,分离外周血单个核细胞(PBMC),用流式细胞术(FCM)检测CD19+B、CD5+CD19+B、CD1d(hi)CD5+CD19+B细胞亚群的比例变化;用ELISA法测定PBMC培养液上清中白细胞介素-10(IL-10)及转化生长因子-β1(TGF-β1)水平,与同期选择的健康人对照组比较。 结果:与对照组相比,ITP患者外周血中CD19+B、CD5+CD19+B细胞亚群的比例均升高,而CD1d(hi)CD5+CD19+ Breg的比例降低,差异均有统计学意义(P<0.05)。ITP患者PBMC培养液上清IL-10、TGF-β1的水平低于健康人对照组(P<0.05)。患者Breg的比例与IL-10表达水平呈正相关(r=0.717,P<0.05),而与TGF-β1水平无相关性(r=0.073,P>0.05)。 结论:CD1d(hi)CD5+CD19+ Breg及相关细胞因子IL-10的下调可能参与ITP的免疫调节紊乱机制。

    Abstract:

    Abstract: Objective:To study the changes of regulatory B cells (Breg) subsets in peripheral blood of patients with idiopathic thrombocytopenic purpura (ITP) and related cytokines, analyze their correlation and explore the role of Breg in the pathogenesis of ITP. Methods:The proportional changes of CD19+B, CD5+CD19+B and CD1d(hi)CD5+CD19+B subsets in peripheral blood were determined by flow cytometry (FCM) and the levels of IL-10 and TGF-β1 in the supernatant of culture medium of peripheral blood mononuclear cells (PBMC) were measured by ELISA in 30 patients with ITP and 30 matching healthy controls. Results:Compared to normal controls, the proportions of CD19+B, CD5+CD19+B subset were increased, but the proportion of CD1d(hi)CD5+CD19+B subset was decreased, and there were statistically significant differences (P<0.05). The expression levels of IL-10 and TGF-β1 in culture medium of PBMC of ITP patients were lower than those in healthy controls (P<0.05). There was a positive correlation between the proportion of CD1d(hi)CD5+CD19+B subsets and the levels of IL-10(r=0.717,P<0.05), but there was no correlation between CD1d(hi)CD5+CD19+B subsets and the levels of TGF-β1 (r=0.073, P>0.05). Conclusion:The low expression of CD1d(hi)CD5+CD19+B subsets and related cytokines IL-10 may participate in the mechanism of immunoregulative dysfunction in ITP patients.
