    目的 探讨乙型肝炎病毒相关性肝硬化(HBV-LC)患者外周血自然杀伤细胞(NK)和自然杀伤T细胞(NKT)功能相关分子表达特点及意义。方法 收集代偿期HBV-LC患者(HBV-CLC)、失代偿期HBV-LC患者(HBV-DLC)和同期体检健康者(HC)各25例作为研究对象。流式细胞术检测各组外周血NK和NKT细胞比例、数量变化,活化标志物(CD69、HLA-DR)、激活性受体(NKG2D、NKp30、NKp46)和抑制性受体(NKG2A、CD158a、CD158b)以及细胞毒分子[穿孔素、颗粒酶A/B、肿瘤坏死因子相关凋亡诱导配体(TRAIL)、IFN-γ]表达。结果 与HC和HBV-CLC患者比较,HBV-DLC患者外周血NK细胞比例和数量均显著降低(P<0.01),但高表达HLA-DR(P<0.01);与HC患者比较,HBV-DLC患者外周血NKT细胞比例、数量均显著降低(P<0.05),但高表达CD69和HLA-DR(P<0.05);此外,相比于HC,HBV-CLC患者外周血NKT细胞比例降低(P<0.05),但数量和活化标志物的表达差异并无统计学意义(P>0.05)。各组间抑制性受体和大部分激活性受体(NKG2A、CD158a、CD158b和NKp30、NKp46)的表达差异无统计学意义(P>0.05);相比于HC,HBV-LC患者外周血NK细胞NKG2D表达显著降低而NKT细胞毒分子(穿孔素、颗粒酶A、TRAIL、IFN-γ)表达显著升高(P<0.05),HBV-DLC患者外周血NK细胞部分细胞毒分子(穿孔素、TRAIL)表达升高(P<0.05);相比于HBV-CLC患者,HBV-DLC患者外周血NK细胞穿孔素表达减少(P<0.05),NKT细胞虽NKG2D表达升高但数量和IFN-γ表达均降低(P<0.05)。此外,相比于HC和HBV-CLC患者,HBV-DLC患者外周血NK细胞杀伤能力受到显著抑制(P<0.01)。结论 HBV-LC患者尤其HBV-DLC患者外周血NK和NKT细胞功能特点发生显著改变,或许能为HBV-LC患者免疫防治提供新的视角和理论基础。

    Abstract:

    Objective To investigate the expression characteristics and significance of function-related molecules of natural killer (NK) cells and natural killer T (NKT) cells in peripheral blood of patients with hepatitis B virus-related liver cirrhosis (HBV-LC). Methods Peripheral blood samples from 25 patients with compensated HBV-LC (HBV-CLC), 25 with decompensated HBV-LC (HBV-DLC) and 25 healthy controls (HC) in the same period were collected, and the proportion and quantity of NK and NKT cells in peripheral blood and the expression levels of activation markers such as CD69 and HLA-DR, activating receptors such as NKG2D, NKp30 and NKp46, inhibitory receptors such as NKG2A, CD158a and CD158b and cytotoxic molecules such as perforin, granzyme A/B, tumor necrosis factor related apoptosis-inducing ligand (TRAIL) and IFN-γ of peripheral NK and NKT cells were detected by flow cytometry. Results Compared with HC and HBV-CLC patients, the proportion and quantity of NK cells in peripheral blood of HBV-DLC patients were significantly reduced (P<0.01), while the expression levels of HLA-DR were increased obviously (P<0.01). Compared with HC, the proportion and quantity of NKT cells in peripheral blood of HBV-DLC patients were significantly reduced (P<0.05), while the expression levels of CD69 and HLA-DR were increased obviously (P<0.05). In addition, the proportion of NKT cells in peripheral blood of HBV-CLC patients was significantly lower than that of HC (P<0.05). However, there were no significant differences in the quantity of NKT cells and expression levels of activation markers between them (P>0.05). There were no significant differences in the expression levels of inhibitory receptors (NKG2A, CD158a and CD158b) and most activating receptors such as NKp30 and NKp46 among the HBV-CLC, HBV-DLC and HC groups (P>0.05). Compared with HC, the expression levels of NKG2D of NK cells in peripheral blood of HBV-LC patients were significantly reduced, while the expression levels of cytotoxic molecules such as perforin, granzyme A, TRAIL and IFN-γ of NKT cells in peripheral blood were significantly increased (P<0.05). The expression levels of some cytotoxic molecules such as perforin and TRAIL of NK cells in peripheral blood of HBV-DLC patients were increased obviously (P<0.05). Compared with HBV-CLC patients, the expression levels of perforin of NK cells in peripheral blood of HBV-DLC patients were reduced (P<0.05), while the expression levels of NKG2D of NKT cells were increased and the quantity of NKT cells and the expression levels of IFN-γ were decreased (P<0.05). Compared with HC and HBV-CLC patients, the killing ability of NK cells in peripheral blood of HBV-DLC patients was significantly inhibited (P<0.01). Conclusion The function characteristics of NK and NKT cells in peripheral blood of HBV-LC patients, especially HBV-DLC patients, have been changed significantly, which may provide a new perspective and theoretical basis for the immune prevention and treatment of HBV-LC patients.
