    目的 评估分枝杆菌基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)鉴定与多靶位基因测序结果的一致率,为MALDI-TOF MS鉴定分枝杆菌的临床应用提供理论依据,进一步了解当地的分枝杆菌菌种分布。方法 将潍坊市第二人民医院2016年7月到2017年10月期间经MALDI-TOF MS(生物梅里埃公司)鉴定的751例分枝杆菌菌株进行RNA聚合酶β亚基(rpoB)、热休克蛋白65(hsp65)、16S rRNA 基因的部分序列测序,以3个靶基因联合鉴定的结果作为“金标准”,评估MALDI-TOF MS对分枝杆菌鉴定的准确度,同时分析近5年临床分离的6350株分枝杆菌MALDI-TOF MS鉴定的菌种分布情况。结果 以rpoB、hsp65、16S rRNA测序鉴定结果为参考,MALDI-TOF MS(生物梅里埃公司)对结核分枝杆菌复合群鉴定的准确率为99.66%,对非结核分枝杆菌(non-tuberculosis Mycobacteria,NTM)鉴定的准确率为97.40%。近5年潍坊地区临床分离的分枝杆菌中NTM占15.51%,NTM种类主要为缓慢生长型(90.46%),其中胞内分枝杆菌占比最高(73.20%),其次为堪萨斯分枝杆菌(12.08%);快生长型分枝杆菌以脓肿分枝杆菌为主(64.90%)。结论 MALDI-TOF MS鉴定分枝杆菌结果可靠,具有重要的临床应用价值;潍坊地区NTM以胞内分枝杆菌为主。

    Abstract:

    Abstract: Objective To evaluate the consistency of mycobacteria identification by the matrix -assisted laser desorption/ ionization time-of- flight mass spectrometry ( MALDI-TOF MS) and multilocus sequencing, and then provide a theoretical basis for the clinical application of MALDI-TOF MS ( BioMerieux,IVD V3.0) in identifying mycobacteria and further understand the local distribution of mycobacteria. Methods The RNA polymerase β subunit ( rpoB),heat shock protein 65 ( hsp65) and 16S rRNA partial sequences of 751 strains of mycobacteria identified by MALDI-TOF MS in Weifang No. 2 People' s Hospital during July 2016 and October 2017 were identified by the multilocus sequencing. The results of joint identification of the three genes were used as the gold standard to evaluate the accuracy of mycobacteria identification by MALDI-TOF MS. Meanwhile, the distribution of 6 350 strains of mycobacteria isolated from clinical samples in the past 5 years and identified by MALDI-TOF MS was analyzed. Results Referring to the sequencing results of rpoB, hsp65 and 16S rRNA, the identification accuracies of MALDI-TOF MS were 99.66% for M. tuberculosis complex and 97.40% for non-tuberculosis mycobacteria ( NTM),respectively. In the past 5 years, 15.51% of clinically isolated mycobacteria were NTM in W eifang region, and the main NTM species were slow-growing mycobacteria ( 90.46%),of which M. intracellulare accounted for the highest proportion (73.20% ),followed by M. kansasii ( 12.08% ). Most of the fast-growing mycobacteria were M. abscessus (6.19%).Conclusion MALDI-TOF MS is reliable in the identification of mycobacteria and has important clinical value. The NTM in Weifang region is dominated by M. intracellulare.
