    摘要:目的建立一种基于过柱法核酸提取的离心式微流控技术用于常见生殖道感染病原体检测的方法。方法设计特异性引物探针,并将过柱法核酸提取+qPCR扩增检测整合于一张离心式微流控芯片上,实现对9种常见生殖道病原体(人型支原体、单纯疤疹病毒型、沙眼衣原体、解脲脲原体、阴道毛滴虫、淋病奈瑟菌、单纯疤疹病毒I型、生殖道支原体、苍白螺旋体)的全自动检测,评价该方法的最低检出限、特异性、重复性和线性关系;检测255例阳性和45例阴性临床标本,验证准确性。结果离心式微流控芯片法的总体最低检出限为500copies/mL;与常见生殖道病原体无交叉反应;精密度总CV均<5%;具有良好的线性关系(R2均>0.99)。与常规方法比较,离心式微流控芯片法的灵敏度、特异性、阳性预测值、阴性预测值和总符合率均高于96.2%,两种检测方法在常见9种生殖道病原体的差异无统计学意义(P均>0.05),具有极好的一致性(Kappa值均>0.96)。结论基于过柱法核酸提取的离心式微流控技术在检测常见生殖道感染病原体中表现出优异的灵敏度、重复性、特异性和准确性,具有重要的临床应用潜力。

    Abstract:

    Abstract: ObjectiveTo establish a detection method for common pathogens causing reproductive tract infections using centrifugal mi-crofluidic technology based on the column for nucleic acid extraction in column. MethodsThe specific primer probes were designed and column-based nucleic acid extraction combined qPCRamplification detection were integrated onto a centrifugal microfluidic chip to achieve fully automated detection of 9 common reproductive tract pathogens Mycoplasma hominis,human herpes simplex virus,Chlamydia trachomatis,Ureaplasma urealyticum,Trichomonas vaginalis,Neisseria gonorrhoeae,Herpes simplex virusI,Mycoplasma genitalium,and Treponema pallidum. The minimum detection limit,specificity,repeatability and linearity of this method were evalua- ted, and 255 positive and 45 negative clinical specimens were tested to verify the accuracy. Results The minimum detection limit of the centrifugal microfluidic chip was500 copies/mL. No cross-reactivity with common reproductive tract pathogens was found. The pre-cision was less than 5%,and good linearity was shown(R2>0.99). Compared with the conventional detection methods,the sensitivity,specificity,positivepredictivevaluenegativepredictivevalueandoverallcoincidencerate of thecentrifugalmicrofluidicchipwereall higher than 96.2%. There was no statistically significant difference between the results of the two methods in the detections of 9 common reproductive tract pathogens(P>O.05),indicating excellent consistency(all Kappa value>0.96).ConclusionThe centrifugal mi-crofluidic technology based on the nucleic acid extraction using the column has shown excellent sensitivity,repeatability,specificity and accuracy for detectingcommonpathogensin reproductivetractinfection,indicating significantclinical application potential.
