    摘要:目的 探讨基质辅助激光解吸电离飞行时间质谱技术(MALDI-TOF MS)鉴定诺卡菌最优前处理方法。方法 对临床分离的93株诺卡菌应用MALDI-TOF MS进行鉴定,采用双甲酸法、玻璃珠震荡-甲酸-乙腈法、原位甲酸法3种不同前处理方法,以16S rRNA、secA1基因测序为参考方法进行结果评价。运用Kappa统计量进行统计学一致性分析。结果 93株诺卡菌中,91株(97.8%)经双甲酸法鉴定至种,88株(94.6%)经玻璃珠震荡-甲酸-乙腈法鉴定至种,而只有64株(68.8%)经原位甲酸法鉴定至种;与基因测序相比,种鉴定能力Kappa值分别为0.975、0.936、0.652。对于脓肿诺卡菌复合群,双甲酸法、玻璃珠震荡-甲酸-乙腈法、原位甲酸法3种方法鉴定至种的准确率分别为95.5%(21/22)、86.4%(19/22)、36.4%(8/22),与基因测序相比,Kappa值分别为0.900、0.730、0.251。结论 双甲酸法操作简单、快速,鉴定准确率高。对于脓肿诺卡菌复合群,双甲酸法鉴定准确性优于玻璃珠震荡-甲酸-乙腈法。推荐在MALDI-TOF MS鉴定诺卡菌中采用双甲酸法进行前处理。

    Abstract:

    Abstract: ObjectiveTo investigate the optimal pretreatment method for the identification of Nocardia using matrix assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOF MS). MethodsAtotal of 93 clinical isolates of Nocardia were identified by MALDI-TOF MS using three pretreatment methods: diformic acid method, glass bead oscilation-formic acid-acetonitrile extraction, and in-situ formic acid extraction. The results were evaluated with gene sequencing results,including 16S rRNA and secAl gene sequencing as the reference method. Statisticalagreement was assessedusingKappa statistics. ResultsAmong the 93Nocardia strains,91 (97.8%)were successfully identified at the species level using the diformic acid method, 88(94.6%)using the glass bead oscillation-formic acid-acetonitrile extraction method,and 64 (68.8%)using the in-situ formic acid extraction method. Compared with genesequencingtKappavaluesfospecies-lvlidentifcationcapabilitwere.andrespectivelyFortheNocardia abscessus complex,the species-level identification accuracies of the three methods were 95.5%(21/22),86.4%(19/22),and 36.4% (8/22),respectively,with corresponding Kappa values of O.900,0.730and 0.251. ConclusionThe diformic acid method is simple, rapid, and highly accurate. For the Nocardia abscessus complex, this method demonstrated higher identification accuracy than the glass bead oscillation-formic acid-acetonitrile extraction method. It is recommended to use the diformic acid method for pretreatment in MALDI-TOF MS identification of Nocardia.
