    摘要:目的 分析高黏液表型肺炎克雷伯菌(hypermucoviscous Klebsiella pneumoniae, hmKP)的耐药特征、生物被膜形成能力及荚膜血清型分布情况。方法 收集2024年1—12月广东省人民医院临床分离的hmKP菌株40株,并同期随机选取经典型肺炎克雷伯菌(classic Klebsiella pneumoniae, cKP)菌株40株。采用Vitek 2 Compact系统进行药敏试验,应用结晶紫染色法检测菌株在2 h、4 h、6 h的生物被膜形成能力,通过PCR扩增wzi基因并测序进行荚膜分型。比较两组菌株的耐药性、生物被膜形成能力及荚膜型别分布差异。结果 hmKP菌株主要来源于呼吸道(16株,40.0%)、血液(10株,25.0%)及尿液(8株,20.0%);hmKP对常用抗菌药物保持较高敏感性,未检出碳青霉烯类耐药菌株;生物被膜检测显示,培养2 h时hmKP组A540 nm/A620 nm比值(0.18±0.10)低于cKP组(0.28±0.22),差异有统计学意义(P=0.019),而4 h及6 h时两组差异无统计学意义(P>0.05);基因分型结果显示,hmKP以K2(14株,35.0%)和K1(12株,30.0%)型为主,cKP组血清型分布相对分散;hmKP组患者预后不良发生率(24例,60.0%)显著高于cKP组(16例,40.0%)(P=0.003)。结论 hmKP对常用抗菌药物保持较高敏感性;与cKP相比,hmKP在荚膜型别分布及早期生物被膜形成方面呈现一定的特征性,且患者预后不良发生率高。

    Abstract:

    Abstract: ObjectiveTo investigate the characteristics of antimicrobial resistance,biofilm-forming capacity,and the distribution of capsular serotype in hypermucoviscous Klebsiella pneumoniae(hmKP). MethodsA total of 40 strains of hmKP isolated from Guang-dong Provincial People's Hospital betweenJanuary and December 2024 were colected. Meanwhile,4O strains of classic Klebsiella pneumoniae (cKP) during the same period were randomly selected as controls. The antimicrobial susceptibility testing was performed using the Vitek 2 Compact system. The biofilm-forming capacity of the strains at 2 h, 4 h, and 6 h was assessed by the crystal violet staining method. The wzi gene was amplifed by PCR and sequenced for capsular genotyping. The differences in antimicrobial resistance,biofilm-forming capacity, and the distribution of capsular types between hmKP and cKP were compared. ResultsThe main sources of hmKP isolates were respiratory tract(16 strains,40.0%),blood(10 strains,25.0%),and urine(8 strains,20.0%). hmKP isolates exhibited high susceptibility to commonly used antimicrobial agents, and no carbapenem-resistant isolates were detected. The biofilm-forming assay showed that the As4o/Aczo.mratios at 2 hours of cultivation in the hmKP group (O.18±O.10) was significantly lower than that in the cKP group (O.28±0.22,P=O.019). However, there was no statistically significant difference between the two groups at 4 and 6 hours(P>0.05).The capsular genotyping results showed that the main subtypes of hmKP were K2(14 isolates,35.0%) and K1 (12 isolates,30.0%),while the serotype distribution of cKP were relatively dispersed. The incidence of por prognosis in the hmKP group (24 cases,60.0%)was significantly higher than that in the cKP group(16 cases,40.0%,P=0.003).ConclusionhmKP remains high susceptibility to commonly used antimicrobial agents. Compared with cKP, hmKP exhibits distinct characteristics in the distribution of capsular types and early biofilm formation and is associated with a higher incidence of poor prognosis.
